Type of Coverage: The Chicago School offers dental insurance to full-time employees through Metlife. Employees may use any dentist they wish, but will save money by using dentists in the network. Participating dentists are listed on the Metlife website, www.metlife.com/dental.
Effective Date: Coverage will be effective on the first of the month following the first date of eligibility.
Enrollment Period: Upon eligibility an employee has 30 days to join the plan or must wait until our yearly open enrollment period, which takes place every October.
Enrollment Procedures: At the time of enrollment, an employee must submit a completed application to the Office of Human Resources.

- Individual Deductible: $50
- Family Deductible: $150
- Class I Preventative: 100%
- Class II Basic: 80%
- Class III Major: 50%
- Annual Maximum: $2000
- Class IV Orthodontia: $1000 Lifetime max (Child Only)
Out-of-Network Coverage Summary:
- Individual Deductible: $75
- Family Deductible: $225
- Class I Preventative: 100%
- Class II Basic: 60%
- Class III Major: 50%
- Annual Maximum: $1250
- Class IV Orthodontia: $1000 Lifetime max (Child Only)
Cost to Employees: Effective November 1, 2008 the employee’s monthly cost for dental insurance will be:
