Health Insurance

Type of Coverage: The Chicago School offers medical insurance to full-time employees through Blue Cross/Blue Shield of Illinois.  We offer both an HMO and a PPO. (Part-time faculty employees who have been continuously employed since on or before January 1, 2000 remain eligible to participate in health insurance benefits.)

Effective Date:  Coverage will be effective on the first of the month following the first date of eligibility.

Enrollment Period:  Upon eligibility an employee has 30 days to join the plan or must wait until our yearly open enrollment period, which takes place every October. 

Enrollment Procedures:  At the time of enrollment, an employee must choose between the PPO and HMO options, and submit a completed application to the Office of Human Resources.  If the HMO option is chosen, a physician must also be chosen at the time of enrollment.  Up-to-date provider lists can be found on the Blue Cross website, www.bcbsil.com.

Benefits: 

HMO Coverage Summary:

  • Individual Out-of-Pocket: $1,500
  • Family Out-of-Pocket: $3,000
  • Emergency Care: $50
  • Office Visit: $10

PPO In-Network Coverage Summary:

  • Individual Deductible: $250
  • Family Deductible: $750
  • Individual Out-of-Pocket: $500
  • Family Out-of-Pocket: $1,500
  • Emergency Care: $50 Copay, then 100%
  • Office Visit: $10 Copay

PPO Out-of-Network Coverage Summary:

  • Individual Deductible: $500
  • Family Deductible: $1,500
  • Individual Out-of-Pocket: $1,500
  • Family Out-of-Pocket: $4,500
  • Emergency Care: $50 Copay, then 100%
  • Office Visit: 70% after deductible

Prescription Drug Retail Copay:

  • Copayments are for up to a 34-day supply at a contracting retail pharmacy
    • Generic: $10
    • Formulary Brand: $20
    • Non-formulary brand: $35

Prescription Drug Mail Service:

  • Maintenance medications are available for up to a 90-day supply and are subject to appropriate copayment amount
    • Generic: $20
    • Formulary Brand: $40
    • Non-formulary brand: $70

Domestic Partners:  The employee and domestic partner must meet the eligibility requirements and have a Recognition of Domestic Partner form on file in the Department of Human Resources.
