Vision Insurance

Type of Coverage:  The Chicago School offers vision insurance to full-time employees through VSP.  Employees may use any provider they wish, but will save money by using providers in the VSP network.  Participating providers are listed on the VSP website, www.vsp.com.

Effective Date:  Coverage will be effective on the first of the month following the first date of eligibility.

Enrollment Period:  Upon eligibility an employee has 30 days to join the plan or must wait until our yearly open enrollment period, which takes place every October. 

Enrollment Procedures:  At the time of enrollment, an employee must submit a completed application to the Office of Human Resources.

Benefits:  

In-Network Coverage Summary:

  • Exam: $10 Copay, every 12 months
  • Single, Bifocal, Trifocal, or Lenticular Lenses: $25 Copay
  • Frames: Up to $120
  • Contact Lenses: your $120 allowance applies to the cost

Out-of-Network Coverage Summary:

  • Exam: Up to $25
  • Lenses: 
    • Single: Up to $30
    • Bifocal: Up to $35
    • Trifocal: Up to $45
    • Lenticular: Up to $60
  • Frames: Up to $45
  • Contact Lenses: up to $105

Cost to Employees:  Effective November 1, 2007 the employee’s monthly cost for vision insurance will be:

 Employee only $3.40
 Employee and Spouse $5.45
 Employee and Children $5.56
 Family $8.97
